Service Re-do Policy: I strive to offer my guests the highest level of guest satisfaction. If you are having challenges with your cut or color, let me know within 7 days of your visit and I will be happy to correct the issue. Cancelation Policy: In order to continue providing the best scheduling options to my guests, I do require 48 hours notice should you need to cancel or reschedule your visit with me. Should you cancel your appointment with less than 48 hours notice, a $25 per service cancelled fee does apply and will be due before a new appointment is scheduled. Allergies: Some individuals can experience an adverse reaction due to the application of hair color and/or stylist products. While rare, symptoms can include burning, redness, itching and/or swelling. Due to the variety of ingredients used in these products, these symptoms may occur even if you have had your hair colored in the past with no reaction. I can administer a "patch test" to determine if you will experience a reaction to a product in the salon. This test must be administered 48 hours prior to your appointment. You can request a patch test at no additional charge to you. Informed Consent: Initialing below indicates that you have read and understand this form, that you have been offered a patch test, and that you are deciding to proceed with your services. Initialing below indicates your consent and agreement to indemnify, defend and holds harmless Fringe Salon and its stylists from any liability claim or action arising from the application of products in the salon.
